1. From the main portal, select the "+New Support Ticket" link near the top right corner of the page.


  2. Or, from the dashboard after you've logged in, click the "+New" button at the top right corner of the page. 
  3. Complete the required form fields, and any applicable optional fields: 
    • Your Email: if you're logged in already, this field will prepopulate with the email you used to register. This is the email where we will contact you regarding your request.
    • Contact Phone: please provide us with a phone number where we can reach you if we have questions.
    • Subject: a brief description of your request
    • Output requested: please select from the drop down list the closest description of the output you require, use "Other" if you're not certain or if none of the list applies
    • Purpose of request: please select from the drop down list the closest description of what you need the output for, use "Other" if you're not certain or if none of the list applies
    • Have you requested these data previously?: please indicate if you have previously requested these data for this purpose, we may already have a program created and just need to edit it for the current year. 
      • If you have previously requested, please let us know about when the last request was submitted. 
    • Staff member you wish to work with: please indicate if there is a particular staff member with whom you wish to work, we will do our best to honor this request.
    • Desired date of completion: please let us know by when you need the completed output. We will work with you to ensure your deadlines are met.
    • Does your request require an IRB?: please check if your request requires an IRB. 
    • IRB Number: please enter the IRB number from your approval letter. To find out how to get a IRB review, click here
    • Please provide us with more details regarding your request: enter specific details regarding the nature of your request and the data and/or reports you require. 
    • Attach: attach any documents or files you feel we may need from you to complete your request. 
    • Submit: click the submit button. We'll respond to you promptly.
